My problem is when I accelerate from a dead stop and in 1st gear, the car revs really high (around 5k) and I'm only going 10 mph. Is this a sign of the clutch slipping? All other gears it's fine, just 1st is the problem. Also, in first, the car jerks violently. Could be a vaccum leak?

Jerking is almost always becaue of a hole in the TB boot. You said that the TB boot was not on tight last time....did you take it off and inspect it? There is probably a crack on the underside of it. If it hasn't been replaced for the entire life of the car, I would bet money that it's cracked or the plastic resonator box is cracked.

As for the slipping clutch, that sounds like a possibility.
